The downtown eastside has poverty, homelessness, and drugs, the likes of which you could not imagine happens in a city in Canada. If you drive down East Hastings Street, it seems like you are on a movie set as it is sooo hard to believe the number of zombie like people walking around (drugs), setting up places to sleep etc. If you google "hastings street" on youtube a few people have videoed it. How they were going to hide it from the rest of the world during the olympics was a huge source of discussion in vancouver. A few years ago it was discovered the area had Hepatitis & HIV infection rates higher than many 3rd world countries. What happens, is that many Canadian junkies migrate to Vancouver, as it's the only city in Canada with weather that allows you to survive on the streets.
